What is the Certificate of Representation?


Writer’s note: This blog series is meant to act as responses to “quick” questions you may have for attorneys.  Think of these answers as a “quick” answer you might receive over coffee from your friendly neighborhood attorney.

A Certificate of Representation is a simple form required at the beginning of a divorce, custody, parenting time, or child support proceeding.  It simply gives the court contact information for the parties, such as address, telephone, and email.  If you are represented by an attorney, your attorney’s contact information will be under your name.  If you are not represented by an attorney, your personal contact information will be under your name.

It is important to put accurate information on this form, as that is where court notices, orders, and other correspondence will be sent.  If you move during your proceeding, make sure to update the court with your new information.
